What Is Sonography Work Really Like?

What is the SPI exam?

SPI stands for Sonography Principles and Instrumentation . It is the physics and instrumentation examination used in ARDMS pathways for RDMS, RDCS, RVT, and RMSKS credentials.

Passing SPI alone does not create a complete professional credential. ARDMS candidates generally pair it with the appropriate specialty examination under a valid prerequisite route.

  • Accreditation evaluates an education program.
  • Certification or registration evaluates an individual against a credentialing organization’s requirements.
  • Licensure is legal permission from a state to practice a regulated occupation.

As of July 2026, the Society of Diagnostic Medical Sonography identifies New Hampshire, New Mexico, North Dakota, and Oregon as the states with legislation mandating licensure of sonographers.

Rules can differ by state and change over time. Verify the current requirements through the state agency before relying on a summary. Oregon, for example, regulates sonography through its Board of Medical Imaging and requires an active state license or permit for covered medical-imaging practice.

The absence of state licensure does not make credentials irrelevant. Employers may require or prefer certification in the specialty being performed. AIUM’s current position is that sonographers should be credentialed in the specialty areas in which they practice.

Certification is not a national license, and one credential does not authorize every specialty everywhere.

Sonographer vs. Radiologic Technologist

Both careers involve diagnostic imaging and close patient contact, but they use different technologies and follow different education and credential pathways.

A radiologic technologist may bring transferable anatomy, patient-care, and imaging experience to sonography. That experience does not remove the need for an appropriate sonography education, supervised clinical preparation, and valid credential eligibility.

ARRT discontinued its postprimary pathway for the general Sonography credential in January 2020. It still offers postprimary pathways for disciplines including Breast Sonography and Vascular Sonography under their current requirements.

Sonographer vs. Cardiovascular Technologist

The boundaries become messier around cardiac and vascular imaging.

A general diagnostic medical sonographer may focus on abdominal, OB/GYN, pediatric, breast, or related imaging.

A cardiac sonographer acquires echocardiographic information about the heart.

A vascular sonographer or vascular technologist evaluates blood vessels and blood flow.

A cardiovascular technologist may work in noninvasive imaging, invasive cardiac catheterization, electrophysiology, or other cardiovascular procedures, depending on the role.

Not every cardiovascular technologist is a sonographer, and not every sonographer works in cardiovascular care. Training and credentials should match the actual job.

Can Sonographers Specialize or Advance?

Experienced sonographers may add specialties, move into leadership, or transition into roles that use clinical expertise differently.

Will AI Replace Sonographers?

Current technology is more likely to change particular tasks than eliminate the occupation in the near term.

FDA-authorized devices already include software that guides image acquisition or optimization, and AI-enabled ultrasound systems can assist with measurements, image quality, and workflow. Tele-ultrasound can also support remote guidance in some settings. These tools do not remove the need for patient interaction, positioning, real-time anatomical adaptation, skilled image acquisition, and communication with the clinical team.

Technology may reduce some repetitive steps and change what new sonographers need to learn. It may also expand ultrasound use into more settings. Long-term staffing effects remain uncertain, so confident predictions in either direction are mostly marketing with a lab coat.

Is Diagnostic Medical Sonography a Good Career for You?

Sonography can be a strong career for somebody who wants technical healthcare work, detailed anatomy, visual problem-solving, and substantial patient interaction. The occupation offers several specialties and is commonly entered through an associate-degree or postsecondary-certificate pathway rather than requiring medical school or a four-year degree for every entry role.

It is not an easy-money shortcut or a low-friction healthcare job.

You should think carefully about the career if you dislike physics, close physical contact, repetitive upper-body work, emotional uncertainty, or clinically demanding education. Hospital work may involve call, weekends, holidays, and very sick patients. Outpatient work can still bring repetitive caseloads and productivity pressure.

A good decision should account for both the upside and the friction:
